IN-HOME CARE
Hospice Brazos Valley's team includes doctors, nurses, chaplains, social workers, volunteers, and hospice care aides to support a patient's and family's physical, emotional, and spiritual needs. However, families or patients occasionally have needs that require an additional layer of support. Services provided by an in-home care agency are highly customizable and
can include short term companion care, 24/7 live-in care, or respite care.

ADVANCED DIRECTIVES
It's never too early or too late to plan for end-of life.
Many advance directive documents are available for free download
Have regular conversations with your attorney, financial planners and family regarding your wishes for your end-of-life.
Documents should be revisited and/or revised every ten years.
